'蔡英文総統閣下': 安倍首相

My translation: The China authority called president of Taiwan "leader of Taiwan authority," and does not recognize/ approve/ accept 総統, which means 大統領. * * * 台北駐日経済文化代表処's home page publicized the message from prime minister, whose opening statement was 蔡英文総統閣下.
(i) 林哲平  Teppei HAYASHI (林 is a Japanese surname, with Japanese pronunciation hayashi (its Chinese pronunciation is "rin" -- recall that Romanization of Japanese has letter R but not letter L. 林 and 森 (another Japanese surname)  is not so different in Chinese language (and often used together as a compound noun), but in Japan, the latter means more trees -- and thus bigger.)
(ii) The Chinese pronunciation for kanji 鉄 and 哲 is identical: "tetsu." (But the pronunciation "tsu," when in the middle (ie, not first or last) of a word  is often replaced a silence , whose duration is the same as any other pronounced syllable.
(iii) In Japanese, there is only 大統領 (for various other nations, such as US and South korea), no 総統. Here 総統 is used to quote Taiwanese title. See next.
(iv) Japanese-English dictionary (Jim Breens online dictionary):
* sōtō 総統 【そうとう】 (n): "(1) supreme ruler; generalissimo; (2) president (of Taiwan)"
* daitōryō 大統領 【だいとうりょう】 (n): "president"
* shidōsha 指導者 【しどうしゃ】 (n): "leader; guide; mentor; coach"
* katagaki 肩書 【かたがき】 (n): "title; address (on letter)"
* kakka 閣下 【かっか】 (n): "(hon[orific]) your Excellency; his Excellency; her Excellency"
*  bōtō 冒頭 【ぼうとう】 (n): "beginning; start; outset"
* mima-i 見舞い 【みまい】 (n): "(2) (See 見舞い品) get-well gifts; get-well letters; (3) (See お見舞い申し上げる) expression of sympathy"
* メッセージ is katakana for English noun "message."

(b) 日本安倍首相致蔡總統慰問函. 臺灣日本關係協會, Feb 8, 2018.

(i) 臺灣日本關係協會 is the parent organization based in Taipei and, since 2012, under Taiwan's Ministry of Foreign Affairs (亞東關係協會, the predecessor of 臺灣日本關係協會, was established in 1972 as the white glove of Taiwan's MOFA).. 台北駐日経済文化代表処 is its sole subsidiary organization based in Tokyo, with branches (equivalent to consulates) in other cities of Japan.
(ii) 相關影片(as opposed to 相關圖片) showed a video clip of prime minister writing kanji "台湾加油 安倍晋三." (加油 is not used in Japan.)
(c) 台湾東部で発生した地震を受けた安倍内閣総理大臣によるお見舞いメッセージ. 首相官邸, Feb 8, 2018.

It is not clear whether the 首相官邸 cut off the head and foot of the letter the moment it put it online on Feb 8, or did it a day later after China's protest on Feb 9.
(d) Days earlier news media in Taiwan reported the letter, but were apparently unaware of the title -- hence not reported about it (title) until now.
